What is the distinction between the wellness programs made available by
Healthyroads Inc. and the benefit programs offered by American Specialty Health Plans, Inc. (ASH Plans)
and American Specialty Health Insurance Company (ASH Insurance Company)?

There is no risk associated with the health education programs provided by
Healthyroads, Inc., and these programs are not regulated.
The California Department of Managed Health Care regulates the HMO programs
offered by ASH Plans.
The at-risk benefit programs offered by ASH Insurance are fully regulated by the
Departments of Insurance in states where ASH Insurance Company is licensed and has received regulatory
approval of its forms. ASH Insurance Company benefit programs are available in the following states:
AR, AZ, CA, CO, DC, DE, FL, HI, ID, IL, MD, MI, MO, NE, NV, OH, OK, OR, PA, SC, TX, VA, and WV.

Is there a minimum group size for enrollment in the programs available through
ASH subsidiaries?

AMERICAN SPECIALTY HEALTH INSURANCE COMPANY (ASH Insurance)
At-risk benefit programs: 51 or more eligible employees
AMERICAN SPECIALTY HEALTH PLANS OF CALIFORNIA, INC. (ASH Plans)
HMO benefit programs: 2 or more enrolled employees
HEALTHYROADS, INC.
Wellness programs: 10 or more enrolled employees

What are the underwriting requirements for the benefit programs?

Programs must be offered alongside an employer-sponsored group medical plan.
-
Each employee/dependent enrolled in the employer-sponsored medical plan must
also be enrolled in the program.

The programs may not be offered on a voluntary basis.
-
The employer may elect to exclude all dependents and offer the rider to
employees only (a 30 percent load applies).
-
Employer must contribute at least 75 percent of the employee price.
-
Effective dates are the first of the month.

Are voluntary programs available?
-
Benefit programs offered by ASH Plans and ASH Insurance Company are not available on a voluntary basis.

How do I Enroll A Group

In order to enroll a group, please send the following items to your account
executive.

Binder check for first month's premium/fee

Copy of the most recent bill from the employer-sponsored medical plan

Employee enrollment provided in one of the following ways:

Completed ASHIC/ASHP/Healthyroads enrollment form for each employee

For the broker/consultant (first group only):

How do I complete a Broker of Record Change

In order to complete a Broker of Record change, please submit the following
items to your Account Executive.
